Medicare Primary Care & Wellness with Janet E. Perez, DNP, APRN, AGNP-C is designed primarily for adults age 65 and older who are looking for a proactive, personalized approach to primary and preventive healthcare.

Our focus is on prevention, healthy aging, chronic condition management, medication review, and helping older adults maintain their health, independence, and quality of life.

Medicare Preventive & Wellness Care

Services may include the Welcome to Medicare Preventive Visit (IPPE), Initial and Subsequent Medicare Annual Wellness Visits, Health Risk Assessments, preventive screening and care-gap reviews, medication review and reconciliation, immunization record review and recommendations, Advance Care Planning when appropriate, and lifestyle, nutrition, and healthy-aging counseling.

Primary Care & Chronic Condition Management

Primary care services focus on establishing an ongoing healthcare relationship and supporting the health needs that commonly arise as we age. Visits may include establishing care, follow-up of existing health concerns, medication management, review and monitoring of chronic conditions, preventive care planning, and coordination of care with specialists and other healthcare professionals when clinically indicated.

Chronic Care Management (CCM) may also be available for eligible Medicare patients with qualifying chronic conditions.

Preventive Care + Primary Care

The Welcome to Medicare Preventive Visit and Medicare Annual Wellness Visits focus on prevention and wellness planning and are not comprehensive physical examinations.

If a medical concern requiring evaluation or management is identified during a preventive visit, a separate medically necessary service may be provided and billed when appropriate. Medicare deductible and/or coinsurance may apply to additional services depending on Medicare coverage requirements.

For online scheduling, simply select Medicare Patient Visit. Your reason for visiting, health history, preventive care history, and Medicare eligibility will be reviewed to help determine the appropriate visit and services for your needs.

Our Practice Focus

Drip & Sip Wellness is designed around preventive care, healthy aging, wellness, and ongoing primary care for older adults. We are not an urgent care or walk-in clinic.

Patients experiencing an acute concern that requires immediate evaluation may be directed to an appropriate urgent care, emergency department, or other healthcare facility based on the level of care needed.
