Adult & Geriatric Primary Care with Janet E. Perez, DNP, APRN, AGNP-C focuses on preventive healthcare, healthy aging, and select primary care services for adults and older adults. Our goal is to help patients stay healthy through Medicare wellness visits, preventive screenings, immunizations, medication reviews, chronic care management, and personalized health education.

Services are centered on prevention, early identification of health risks, and evidence-based wellness strategies to support long-term health and independence. Limited acute care services and CLIA-waived point-of-care testing may be available when appropriate.
